This document is a guide for the organs of quality assurance and implementation in Internal Quality Audit (IQA) activities at the faculty, study program, and other governance organ levels which are carried out once a year. The education sector is carried out after the completion of the academic year while the non-academic field is carried out at the end of each fiscal year. There are two types of IQA activities, namely:

1. audit desk, audit activities on independent monitoring-evaluation reports and satisfaction level survey reports;
2. field audit, audit activities in the field based on a list of findings or a checklist that has been compiled on the audit desk activity.

In the IQA activity, the root of the problem, supporting and inhibiting factors, as well as follow-up, and recommendations were formulated.
